LEGAL & FINANCIAL

□  Connect with local Developmental Disability Office for funding support and Targeted Case Management services.

□  Apply for Guardianship/supported decision making and or payee or conservatorship based on need.

□  Re-Apply as adult (age 19) for Medicaid (KanCare), Supplemental Security Income.

EMPLOYMENT & INDEPENDENT LIVING

□  Identify skills for desired job and obtain paid or volunteer employment with supports as needed

□  Practice effective communication by practicing asking for help, developing interview skills, and identifying necessary accommodations.

□  Practice arriving on time to work, appointments, and social activities.

□  Practice taking care of health care needs (making appointments, filling and taking prescriptions, etc.)

□  Attend and visit community based day programs and residential settings.

ADVOCACY

□  Aid your child to be able to explain their disability and their needed accommodations. Encourage your child to make choices and to positively share what they want to help develop self advocacy skills.

□  Connect with other parent/guardians and advocates through the Advocates in Communities Team (ACT) to learn how to share your story to make positive changes for your family and others.

□  Update any and all service and support providers of changes in address, phone and other contact information.
