LEGAL & FINANCIAL

□  Connect with local Developmental Disability Office for funding support and Targeted Case Management services.

□  Check if your child qualifies for Medicaid https://cssp.kees.ks.gov/apspssp/ or SSI https://www.ssa.gov/  ph (800) 772-1213

□  Apply for determined needs of medical and financial support – Medicaid, SSI, and state financial Supplemental programs.  Even if receiving Soc Sec benefits as a minor, must apply for adult benefits – not automatic.

□  Obtain a state identification card (Kansas ID) from the DMV and learn to communicate basic information.

SPECIAL EDUCATION

□  Connect with Families together for education regarding special education and child supports. https://familiestogetherinc.org/

□  Invite care coordinators and/or case managers to all IEP meetings. Connect school & case management with your outside resources to create a team for your child.

□  Explore interests and skills, complete interest and career inventories, and identify additional education or training needs to improve skills..

HOME & COMMUNITY

□  Expect your child to complete household chores: cooking, laundry, dishes, cleaning, etc.

□  Match career interests and skills with vocational course work, community work, volunteer and employment experiences.

□  Practice transportation options outside of the family (Wichita/Para Transit)

□  Assist your child to use Money management and Budgeting skills-Take them shopping and make they pay.

□  Identify and help your child to connect with groups that share similar interests: athletic, faith-based, creative arts, humanitarian, civic, 4-H, and cultural.  Broaden experiences with community activities and expand friendships (AbilityPoint, Circle of Friends, Special Olympics, YESS Camp).

ADVOCACY

□  Connect with other parent/guardians and advocates through the Advocates in Communities Team (ACT) to learn how to share your story to make positive changes for your family and others.

□  Update any and all service and support providers of changes in address, phone and other contact information.
